Office Policies

Please bring the following items with you to your visit: (if they apply)

  • Completed New Patient Form (Demographic information)-we ask that you  fill out the Demographic information form  and bring in to your appointment.
  • Completed Consent for treatment and Consent for release of information (for individual therapist and/or pediatrician or primary care provider, if applicable).
  • Records of any psychiatric evaluation or psychological testing (if previously treated)
  • Payment (deductibles or self-pay).

 

All fees or due balances are to be paid at the time of service (by check, cash or credit card).

All letters that are requested will be subjected to a $25.00-$50.00 charge per letter to be paid at the time of the request.

A comprehensive psychiatric report, requested by parents or schools, will be charged at one session fee.

All prescription refills are to be filled on the date of your follow up appointment. In the event that you miss your appointment, it is very important that you make a new one as soon as possible. This office will only approve enough medication until your next appointment. If a refill is needed, please allow 48 hour advanced notice. Refill requests will be fulfilled during business hours, Monday to Friday.

It is the office policy that credit card information is kept on file to process unpaid balances older than 60 days.

Your appointment time is specifically reserved for you. If you need to cancel or reschedule an appointment, please call at least 24 hours in advance. Failure to attend a scheduled appointment or cancellation with less than 24 hours notice may result in your being  billed for the full appointment fee.
