Please bring the following items with you to your visit (if they apply):

Completed New Patient Form (Demographic information)-we ask that you fill out the Demographic information form and bring in to your appointment.
Completed Consent for treatment and Consent for release of information (for individual therapist and/or pediatrician or primary care provider, if applicable).
Records of any psychiatric evaluation or psychological testing (if previously treated).
